DENGVAXIA®

なんとこれは、世界初のデング熱ワクチンです。世界に先駆けて、メキシコで発売されました。

実は、最近訪問したメキシコ滞在中にこれを知りました。

やたら蚊に刺される体質なので、このワクチンは受けたかったのですが、現地で接種する機会はありませんでした。

もちろんしっかりと蚊には刺されましたが、幸いデング熱やジカ熱には罹りませんでした。

ブラジルやフィリピンでも承認されているようですから、今後更に多くの国でも販売されていくことと思います。

要注目です。

安楽死と薬剤師

Urgent need for guidance on pharmacists’ role in physician-assisted death
というタイトルのニュースがありました。

カナダで安楽死を法的に認めるかどうかの議論が続いています。そして、それが認められた場合、薬を取り扱う薬剤師は当然そのプロセスに巻き込まれていきます。

致死量の薬を調剤し、払い出すにあたって、どのようなガイドラインに則ってその処方せんを取り扱っていくべきかとう話です。もちろん倫理的にという話です。。

アルバータ州では、すでにガイドライン原案が薬剤師協会により作成され、今後詰めの作業に入っていくそうです。

15年前に大学の倫理の授業を受けていたころは、このようなプロセスに薬剤師が関ることになろうとは考えもしませんでした。

最もこのような場合でも、院外処方により薬局で調剤するという原則があてはまるがために、薬剤師の責任は重くなるわけです。

ポイント制の是非

College of Pharmacists of BC wins appeal in incentives case!
というタイトルでカレッジからメールがきました。
以下、本文もコピーします。

The College is pleased to announce that the BC Court of Appeal released its decision today on the bylaws prohibiting the provision of incentives for prescriptions or pharmacy services.

The BC Court of Appeal ruled unanimously in favour of the College of Pharmacists of BC, which means that the bylaws are now in force.
Honorable Madam Justice Newbury's decision states "The College can prohibit incentive programs ... inducements from pharmacists [are] a matter of public interest and professional standards." Read the full decision on the BC Courts website
This decision overturns the BC Supreme Court decision that was released on July 25, 2015. 

The BC Court of Appeal decision is a significant milestone for a self-governing profession and a major achievement since the College first put forth the bylaws on December 2, 2013. 
The bylaws prohibiting the provision of incentives for prescriptions or pharmacy services are found under the Health Professions Act Bylaws - Schedule F - Part I Community Pharmacy Standards of Practice and the Pharmacy Operations and Drug Scheduling Act Bylaws

顧客獲得のために様々な手段がとられるのは、どこの国でも一緒です。
そして方法によっては、裁判にまでかけられて是非を問われることもある、という例でした。

余談ですが、現在のBC州のカレッジのトップは、日系のボブ・ナカガワさんです。

CBC’s ‘Marketplace’ apologizes

要約すると、市販のサプリメントのビタミンC含有量は、ラベルの記載量よりも少ないという話が、昨年CBCマーケットプレースにより特集されました。テレビとインターネットです。

今になって、依頼先のラボの分析結果が間違っていました、ゴメンナサイ、ということです。

一般市民への影響って、どの程度だと思いますか???

実際には、報道内容を気にすることなくビタミンCのサプリを飲んでいる人はたくさんいます。

特に私の地域では、この冬、鼻、咳、耳にウィルスやバクテリアの集中攻撃を受けた人が多く、少しでも免疫力向上の救いになればということでしょう。

ZIKA

中南米でジカウイルスの報告が相次ぎ、WHOも声明をだしています。

これにならって、PUBLIC HEALTH AGENCY OF CANADAや、厚生労働省検疫所FORTHもウェブサイトに情報をアップしています。

この時期、膨大な数の人がカナダから温暖な中南米へ旅行をしますから、最低1日2~3回は虫除けの話をしています。

予防としては、それくらいしか出来ることはありません。今のところは。




そして、人は太陽を求めて旅行しますから、日焼け止めの話もしなければなりません。

DEET含有虫除けの場合、塗る順番としては、日焼け止めが先、虫除けが後です。

Repellents and Sunscreen

Repellents that are applied according to label instructions may be used with sunscreen with no reduction in repellent activity; however, limited data show a one-third decrease in the sun protection factor (SPF) of sunscreens when DEET-containing insect repellents are used after a sunscreen is applied. Products that combine sunscreen and repellent are not recommended, because sunscreen may need to be reapplied more often and in larger amounts than needed for the repellent component to provide protection from biting insects. In general, the recommendation is to use separate products, applying sunscreen first and then applying the repellent. Due to the decrease in SPF when using a DEET-containing insect repellent after applying sunscreen, travelers may need to reapply the sunscreen more frequently.

Nicotine Gum

昨日吸入器の話をしたので、今日はニコチンガムの話をしましょう。

PEBC(OSCE)の受験勉強をしていたころ、これだけはきちんと練習しました。
当時チューターしてくれた一人が、
「言うべき内容は決まっているんだからパッパ、パッパと大事な点をカバーして!」
とハッパをかけてきたからです。
逆にいうと、当時の私はそれだけスローな話し方をしたんですね。
基本的に日本人は英語を話すことが苦手ですから、大事なことは、
「ポイントをおさえて、ゆっくり、ハッキリ」話すことです。
でも、車の運転と一緒で、練習していれば速いスピードで走れるようになります。

それはさておき、ニコチンガムで禁煙したい人には、
「ガムといっても、普通のガムとは違いますからね!」
と、適切な使用方法を説明する必要があり、
そうでもしなければ5分でガムを捨てる人だっています。
1個のガムは15分以上持続するという説明を忘れずに。
以下に、英語の説明を加えておきます。

それにしても毎日少しずつNRTを求めて薬局へ来る人たちをみていると、
潜在的なニーズはあるものだなーと毎日関心しています。
そして誰もが最近は健康志向です。

Advantages
  • The nicotine gum good for occasional smokers to reduce withdrawal symptoms, cravings and manage stress.
  • You can use the gum to help you reduce the number of cigarettes you smoke daily.

How to use
Note: The taste of nicotine gum can be unpleasant at first but most people get use to it with regular use.
  • Nicotine gum shouldn’t be chewed like regular chewing gum. Use bite-park technique. 'Bite' the gum once or twice, then “park” the gum in the corner of your mouth (between your cheek and gums).
  • Wait one minute, bite the gum a few more times, and then “park” the gum again.
  • Repeat the 'bite' and “park” process for 30 minutes.
  • It takes 15 to 30 minutes for the gum to start working so use the gum prior to when you think you will need it. For instance, if you used to smoke on your coffee break, take the gum 30 minutes prior to your regular break.
  • Avoid acidic food or beverages like coffee, tea, juice, and soda 15 minutes before using the gum. The acid in these products can prevent your mouth from absorbing the nicotine from the gum.

Common side effects
Nicotine gum is not like regular gum. If used incorrectly, you are more likely to experience side effects, including
  • Nausea
  • Stomach upset
  • Mouth, throat or gum irritation
  • Jaw ache
  • Hiccups
  • Headaches
  • Trouble sleeping

Nicorette Inhaler

2016年1月1日に州政府による禁煙補助プログラムがアップデートされたのに伴って、結構な数の患者さんがNRT(Nicotine replacement therapy)購入のために薬局へやってきます。

今回公費負担となった商品の一つがこのNicorette Inhalerという商品。
今まであまり需要がなかったので知らないことが沢山ありました、。

指先にタバコを持っている「感覚」が一番のウリだと思いますが、
使用方法を説明するときのポイントは、ノドの奥に向けて吸い込み、
口腔内からニコチンを吸収させること。

するとどうでしょう。

「何だよそれ」といった感じのコメントがよく帰ってきます。
肺に吸い込む代わりに、ニコチンを口にためておくことが現実的ではないのです。

それならば他のオプションはということで、ガムを選ぶ人がほとんどです。

それはそれで患者さんに選択の自由がありますから、
どこまでもよい相談相手になることが大切です。
決して押し付けがましくならないように。

最近、日本でも英語を話さなければいけない場面が増えてきていると思います。
常に「これを英語でどういうか?」と考える訓練が必要ですね。

以下に英語の資料をまとめます。
とりあえず丸暗記して、試験なり、患者相談なりに備えましょう。結局そういう繰り返しです。

Nicotine Inhaler

Advantages
The inhaler is a plastic mouthpiece with a replaceable nicotine cartridge inside, which is sucked on like a cigarette. Unlike cigarettes, inhalers don't deliver a 'hit', but they can help with cravings by releasing nicotine which is absorbed through the lining of the mouth.
  • It mimics the hand-to-mouth action of smoking.
  • The user decides how often to use the inhaler.
  • Reduces the symptoms of nicotine withdrawal cravings and irritability.
Common side effects
  • Nausea
  • Headaches
  • Mouth or throat irritation
  • Stomach upset
How to use
Note: Stop smoking completely before starting the inhaler.
  • Take the mouthpiece out of its plastic wrapper.
  • Line-up the markings on the mouthpiece and separate the two pieces.
  • Remove one cartridge from the foil wrapped tray.
  • Press the cartridge firmly down into the bottom of the mouthpiece until the seal breaks.
  • Replace the top of the mouthpiece. Line up the markings.
  • Press the cartridge firmly down to break the upper seal of the cartridge.
  • Twist the mouthpiece to mis-align the markings.
  • With the tapered end of the mouthpiece in your mouth, either take in long deep breaths or short puffs.
  • Each cartridge will last for about 20 minutes of continuous frequent puffing.
  • When the cartridge is empty, remove the cartridge from the mouthpiece and dispose of the cartridge out of reach from children and pets.
  • Clean the mouthpiece regularly with soap and water.
  • Don't have any acidic beverages like coffee, tea, juice, and soda while using the inhaler. The acid in these drinks can prevent your mouth from absorbing the nicotine from the inhaler.
Recommended schedule
  • Use at least six to twelve cartridges per day initially.
  • Do not use more than twelve cartridges a day.
  • As your body adjusts to not smoking, you can choose to stop using the inhaler or slowly reduce the number of cartridges you use a day.
  • Do not use the inhaler for more than six months.

CP3受講のための条件

CP3に関する質問が続けて寄せられたので、かるくまとめます。

CP3プログラムとは、ブリティッシュコロンビア州立大学が提供する外国人薬剤師のためのブリッジングプログラムで、現在はBC州で免許をとるための必須項目です。CP3受講のための条件は、以下の4点です。

① PEBCの書類審査と試験(EE)にパスしていること
ν DOCUMENT EVALUATION
ν EVALUATING EXAM

② 英語の試験でスコアをとっていること
ν ENGLISH FLUENCY TEST

③ College of Pharmacists of British Columbia (BC州薬剤師協会)へ登録すること
Document Evaluationと同程度の書類の準備が必要になります。心の準備を。

④ ビザがあること(薬局実習のため!)
移民した人の場合、特に問題ありません。
そうでない人の場合、合法的に働くことの出来るビザが必要です。
現実的になオプションとしては以下の2つになります。
a. CP3を通して実習期間(3ヶ月)をカバーできる就労ビザを申請する。
b. ワーキングホリデービザで、実習期間(3ヶ月)をカバーする。

ちなみにCP3受講のために、日本での就労経験や過去の肩書きは特に必要ありません。
薬局でのパート経験しかなくても、薬剤師として30年のキャリアがあっても、大学の教授でも、チェーン薬局社長でも、関係ありません。誰もが同じスタートラインからスタートします。
繰返しますが、CP3の受講に過去の職歴は一切問われません。
