MMRの集団発生に際するカナダの薬剤師的知識と準備

アメリカのアナハイムにあるディズニーランドでMMRが集団発生してから、
バンクーバーのラジオ局がトラベルクリニックで注射をうけるようにと呼びかけています。

このような世界的な感染症の危険を公共的にお知らせする手段として、ヘルスカナダの”Travel health notices”があります。便利なので知っておきましょう。

これはWHOのアップデートに従って更新されていきますが、”Where is measles a concern?”をみてみると、以下の地域に関して特に注意が喚起されています。

  • In the Region of the Americas (North, Central and South America and the Caribbean), measles has been eliminated; however, outbreaks continue to occur when the virus is imported into this region from other countries.
    • The United States is currently reporting a sudden increase in the number of cases of measles in California, which started in December 2014. Related cases have since been reported in other states and in Mexico.
    • Brazil: In 2014, health authorities reported a significant increase in the number of cases as compared to the same time period in 2013.
  • In many countries in other regions of the world (Africa, Asia, Europe, Oceania), measles is present (endemic) and periodic large outbreaks occur. Travellers who are not immune are at risk.
    • Philippines: There have been reported cases of measles in unimmunized travellers who arrived in Canada from the Philippines where there is an ongoing outbreak affecting Manila and other regions of the country.
    • Vietnam is currently experiencing an outbreak; there have been a high number of measles cases reported throughout the country since the beginning of 2014. 
    • China: In 2014, health authorities reported a significant increase in the number of cases as compared to those reported in 2013. Cases of measles have been reported in travellers who arrived in Canada from China.
これに伴い、薬局付設のトラベルクリニックを担当する私もMMRのワクチンを注文することになりました。もっとも、このようなアウトブレイク(集団発生)に備え、政府の支給するワクチンが保健所のような場所に備蓄してあり、そこでも注射が受けられますということをまずは確認しました。

日本育ちの日本人的には、このような感染症のインパクトの実感が湧かないかもしれませんが、とにかく用心してヘルスカナダの指示に従った方が無難です。ご自身が上記の地域に旅行される際には、ぜひ1回注射を受けてください。

ちなみにカナダにはPRIORIX®とM-M-R® IIという2つのブランドがあります。
いずれも原料はMMR成分は「live, attenuated」で、SubQにより投与されます。
Live, attenuatedときた瞬間、何かに気をつけなければ！と思う習慣が必要です。

Priorixの禁忌をみると、

  • subjects with known hypersensitivity to neomycin or to any other component of the vaccine. A history of contact dermatitis to neomycin is not a contraindication.
  • subjects having shown signs of hypersensitivity after previous administration of measles, mumps and/or rubella vaccines.
  • subjects with severe humoral or cellular (primary or acquired) immunodeficiency e.g. symptomatic HIV infection (see also WARNINGS AND PRECAUTIONS).
  • pregnant women. Women of child-bearing potential should be advised to avoid pregnancy for one month following vaccination (see also WARNINGS AND PRECAUTIONS)
と書いてありますから、やはり特別な注意があります。なんといっても今回のアウトブレイクは、ディズニーランドです。ここを訪れるのは子供連れだけでなく、妊婦さんや、これから子供を考えている人は沢山いることと思います。

ところで、MMRとはmeasles, mumps, and rubella（麻疹(はしか)・おたふく風邪・風疹）のことです。Measlesは、その綴りからミーズレスと読みたくなりますが、「ミ」にアクセントを置いて「ミーズォーズ 」と発音されます。http://dictionary.reference.com/browse/measles

どのように感染が拡大するか？と聞かれたら、
When an infected person breathes, coughs, or sneezes, the virus spreads through the air. 
と答えましょう。空気感染や飛沫感染などにより感染します。

もちろんココにはMMRの全てを書ききれませんので、日本語でも以下の国立感染症研究所のサイトを参考にしてよく勉強しておきましょう。

このような健康問題は、規模が大きくなればなるほど、お客さんに質問される回数が多くなります。従って、決してテストのためだけではなく、薬剤師の知識として見につけておく必要があります。もちろん、分からなければその場で調べる!!のが基本です。恥ずかしがる必要はありません。"Please let me look it up"や"I will look it up for you" といえばお客さんは待ってくれますし、あやふやで断片的な知識よりも感謝されますし、何より自分も安心できます。

ちなみに、カルフォルニアはバンクーバーから直行便の飛行機で3時間程度ですから、ホリデーにはもってこいの場所です。アナハイムのディズニーランドは大人でも楽しい場所ですから、みなさん是非遊びに行ってみてください。
